Our Cardiovascular Data Management Center provides comprehensive and continuous quality improvement information and allows us to compare our patient outcomes to state and national benchmarks.</p><p class="MsoNormal"> </p><p class="MsoNormal"><strong>Advanced Practitioner Role Scope </strong></p><ul style="margin-top:0in;" type="disc"><li class="MsoNormal" style="mso-list:l1 level1 lfo1;tab-stops:list .5in;">Nurse Practitioners (APRN) / Physician Assistants provide advanced heart failure consultations within the Skilled Nursing Facility setting throughout the East Region as well as scheduled time in the advanced heart failure outpatient clinic located in Norwich, CT</li><li class="MsoNormal" style="mso-list:l1 level1 lfo1;tab-stops:list .5in;">Assess newly admitted heart failure patients to the SNF setting</li><li class="MsoNormal" style="mso-list:l1 level1 lfo1;tab-stops:list .5in;">Collaborate with other providers on Cardiac Care with the goal of reducing hospital readmissions</li><li class="MsoNormal" style="mso-list:l1 level1 lfo1;tab-stops:list .5in;"> Assist in helping to coordinate discharge providing bridge appointments in the outpatient clinic</li></ul><p class="MsoNormal"><strong>Advanced Practitioners Enjoy</strong></p><p class="MsoNormal">At Hartford HealthCare newly graduated and experienced providers are provided with the support and mentorship they need to succeed through on the job, classroom, and procedural training. <p class="MsoNormal"><strong>Advanced Practitioner Requirements</strong></p><ul style="margin-top:0in;" type="disc"><li class="MsoNormal" style="mso-list:l0 level1 lfo3;tab-stops:list .5in;">Graduate of an accredited Physician Assistant or Nurse Practitioner program</li><li class="MsoNormal" style="mso-list:l0 level1 lfo3;tab-stops:list .5in;">Licensed, or eligible to be licensed, as a Physician Assistant, or Advanced Practice Registered Nurse (APRN) in the state of Connecticut</li><li class="MsoNormal" style="mso-list:l0 level1 lfo3;tab-stops:list .5in;">Certification or eligible to be certified as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse</li><li class="MsoNormal" style="mso-list:l0 level1 lfo3;tab-stops:list .5in;">Cardiology experience required</li><li class="MsoNormal" style="mso-list:l0 level1 lfo3;tab-stops:list .5in;">Geriatric board certification preferred</li></ul><p class="MsoNormal"><strong>We take great care of careers.</strong></p><p class="MsoNormal">With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
