Our collaborative team uses data-driven decisions to continually enhance patient-focused care that is supported by critical thinking, research, education, and innovation in order to continuously promote and improve the musculoskeletal health and well-being of every patient. In this innovative role, the Program Manager is instrumental in implementing and managing strategic initiatives of the Orthopedic/Orthopedic Trauma Service Line. The Program Manager will play a key role in the success of the service line growth and development.</p><p class="MsoNormal"> </p><p class="MsoNormal"><strong>JOB RESPONSIBILITIES</strong></p><p class="MsoNormal">Key areas of responsibility</p><ul style="margin-top:0in;" type="disc"><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">Collaborate with the Physician Service Line leader in addition to Orthopedic and Orthopedic Trauma Surgeons to facilitate Service Line Meetings focused on institute operations, clinical excellence, and Patient Experience.</li><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">Trauma liaison to ensure American College of Surgeons (ACS) guidelines are achieved and sustained.</li><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">Collaborate with Nurse Navigators to optimize patient experience through the care continuum, provide safe patient care through strategic initiatives focused on decreasing preventable patient returns to ED, and conduct real time patient rounding.</li><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">Provide primary oversight of Joint Commission Disease Specific Certification for Fragility Hip Fracture including data validation with the Data Analytics Team.</li><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">Continuous support towards quality and safety initiatives to meet mandatory requirements and regulations (Department of Public Health, CMS and Joint Commission).</li><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">Lead and coordinate Service Line quality improvement initiatives, process improvements, A3s with the multidisciplinary team.</li><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">Onboarding of Orthopedic Residents and new surgeons to include education and overview of guidelines.</li><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">In partnership with the Data Analytics Team supports data quality, coding compliance, and registry requirements.</li><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">Ensure proper Patient Reported Outcomes (PRO) captures on selected virtual platforms.</li><li class="MsoNormal" style="mso-list:l0 level1 lfo1;tab-stops:list .5in;">In alignment with our continuous improvement culture, you may have an opportunity to participate in Hartford HealthCare (HHC) developments.</li></ul><br>

<p class="MsoNormal"><strong>Minimum Requirements</strong></p><p class="MsoNormal"><strong>Education</strong></p><ul style="margin-top:0in;" type="disc"><li class="MsoNormal" style="mso-list:l3 level1 lfo1;tab-stops:list .5in;">Bachelors of Science Degree in Nursing.</li></ul><p class="MsoNormal"><strong>Experience</strong></p><ul style="margin-top:0in;" type="disc"><li class="MsoNormal" style="mso-list:l1 level1 lfo2;tab-stops:list .5in;">At least three years clinical experience in an acute care setting.
