You’ll serve as the central point of contact for patients, families, physicians, and the interdisciplinary care team, ensuring care plans are executed safely, effectively, and with a focus on quality outcomes. The position combines direct and indirect patient care activities and directs staff nurses, LPNs, and CHHAs in delivering the individual patient’s plan of care, as well as identifying interdisciplinary needs and coordinating allied health clinicians.</span></p><p style="margin: 0in;
