<br><b>Overview </b><br>An outpatient primary care organization is seeking a <b>Spanish-speaking Nurse Practitioner </b> to join a value-based clinic in <strong>Brooklyn, New York. </strong><br>This clinic focuses on providing <b>preventative and chronic care management for Medicare patients </b> through a collaborative team model and smaller patient panels.<br>The care model prioritizes <b>patient outcomes, preventative medicine, and stronger provider-patient relationships. </b><br><b>Position Highlights </b><br>• Schedule: Monday Friday<br>• Hours: 8:00 AM 5:00 PM<br>• Setting: Outpatient primary care clinic<br>• Patient population: Primarily geriatric / Medicare<br>• Visit length: 30 minutes<br>• Patient panel capped around 450<br>• EMR: Epic<br><b>Clinical Support </b><br>Each provider receives:<br>• Dedicated medical assistant<br>• Dedicated scribe<br>• Shared RN support<br>• Collaborative physician and APP care teams<br>This structure allows providers to focus on <b>quality patient care rather than high-volume visits. </b><br><b>Responsibilities </b><br>• Provide comprehensive primary care services<br>• Diagnose and treat acute and chronic conditions<br>• Conduct preventative health visits<br>• Educate patients on disease management and lifestyle improvements<br>• Document encounters in Epic EMR<br><b>Requirements </b><br>• Family Nurse Practitioner (FNP) or Adult-Gerontology NP<br>• Board Certified<br>• Active <b>New York NP license </b><br>• Active DEA license<br>• <b>Fluent or professional Spanish proficiency required </b><br>Experience with Medicare or geriatric patient populations preferred.<br>New graduates may be considered.<br><b>Compensation & Benefits </b><br>Compensation is competitive and based on experience.<br>Benefits may include:<br>• Medical / dental / vision insurance<br>• Retirement plan with employer match<br>• CME allowance<br>• Paid malpractice coverage<br>• Paid time off<br>
