You ll focus on early intervention, chronic disease management, and long-term wellness in a setting that values quality over volume. What You ll Do (Key Responsibilities in Patient Care): <ul><li>Comprehensive Care: Provide both in-person and virtual visits to increase access and continuity of care. </li><li>Diagnosis & Planning: Prescribe medications and develop personalized health plans with your patients. </li><li>Wellness Education: Educate patients on wellness, prevention, and lifestyle management. </li><li>Collaboration: Work closely with physicians, medical assistants, and other clinicians. </li><li>Culture: Contribute to a positive, team-oriented clinic culture. </li> </ul>What You ll Bring to the Team (Minimum Qualifications): <ul><li>Active and unrestricted FNP license in the practicing state. </li><li> Board certification as an FNP. </li><li> DEA registration. </li><li>2+ years of primary care experience preferred. </li><li>Experience treating patients ages 2+. </li><li>Familiarity with HIPAA and a strong commitment to patient confidentiality. </li><li>Ability to pass on-site credentialing. </li> </ul>Ready to Make a Meaningful Impact?
