<p> We are currently looking for a <strong>Family Medicine </strong> <strong>nurse practitioner </strong> <strong>AND/OR physician assistant </strong> to join our Family Medicine private practice in <strong>Lincoln, CA </strong> a suburb of Sacramento. </p><p><br></p><p><strong>Key Responsibilities: </strong> </p><ul><li><strong>Comprehensive Primary Care: </strong> Provide preventive care including annual physicals, well-child visits, immunizations, and sick visits for both children (ages 6 and older) and adults. </li><li><strong>Acute Care Services: </strong> Offer urgent care, pediatric care, short-term stabilization, and post-hospital follow-up care designed to keep patients safe, independent, and out of the hospital. </li><li><strong>Chronic Disease Management: </strong> Manage acute and chronic conditions, including providing medication therapy management, diet and nutrition counseling, and mobility/home medical equipment assessments. </li><li><strong>Geriatric & Palliative Care: </strong> Provide specialized adult primary care and patient observation tailored to our senior citizen population. </li><li><strong>Collaborative Practice: </strong> Work collaboratively with our clinical team and network of specialists to ensure comprehensive continuity of care. </li><li><strong>Work Life Balance: Predictable work schedule, flexible full or part-time option, no after hours practice call. </strong> </li> </ul><p><br></p><p><strong>Qualifications: </strong> </p><ul><li> 2+ years of primary care experience required, no new grads considered </li><li> Must love building relationships with patients, value bedside manner, and possess excellent clinical skills </li><li> Must comply with HIPAA rules and regulations </li><li> State licensure and board certification is required </li> </ul><p><br></p><p><strong>About Lincoln, CA : </strong> </p><p> Lincoln, California, has a population of 56,000 residents and is located just outside the <strong>Sacramento </strong> metropolitan area.
