Geriatrics Nurse Practitioner

<p><strong>Nurse Practitioner Long-Term Care New Hampshire </strong> </p><p>We are seeking a Nurse Practitioner to join a facility-based medical team providing comprehensive primary and preventive care to patients residing in skilled nursing and long-term care facilities. This role offers the opportunity to make a meaningful impact on the health and well-being of the elderly population while working collaboratively within an interdisciplinary care team. </p><p><strong>Responsibilities </strong> </p><ul><li><p>Provide primary and preventive care to long-term care residents </p> </li><li><p>Manage residents with acute changes in condition </p> </li><li><p>Perform advanced care planning and chronic care management </p> </li><li><p>Prescribe medications, including controlled substances, in accordance with NH law </p> </li><li><p>Order, review, and follow up on diagnostic testing and treatments </p> </li><li><p>Communicate regularly with nursing facility staff regarding patient condition and care plans </p> </li><li><p>Collaborate with physicians and interdisciplinary care teams </p> </li> </ul><p><strong>Schedule </strong> </p><ul><li><p>Monday Friday, 8:00 AM 5:00 PM EST </p> </li><li><p>4 days per week (schedule coordinated with manager) </p> </li> </ul><p><strong>Qualifications </strong> </p><ul><li><p>Current, unrestricted Nurse Practitioner license in New Hampshire </p> </li><li><p>Board Certified NP </p> </li><li><p>Active DEA license </p> </li><li><p>Background in adult, family, or geriatric medicine </p> </li><li><p>Experience in a long-term care or skilled nursing setting preferred </p> </li><li><p>Proficiency with EMR systems </p> </li><li><p>Strong clinical judgment managing complex, chronic conditions </p> </li> </ul><p><strong>Benefits </strong> </p><ul><li><p>Continuing education credits </p> </li><li><p>Recertification reimbursement </p> </li><li><p>Malpractice insurance </p> </li><li><p>Health, dental, and vision insurance </p> </li><li><p>Paid time off </p> </li> </ul>
