<br><b>Position Overview </b><br>Monogram Health is seeking an experienced Palliative Care Nurse Practitioner to deliver in-home care to a highly complex patient population in the Newark, NJ market.<br>This is a permanent, full-time position focused on end-of-life and serious illness care. The NP will provide comprehensive palliative services, including pain and symptom management, advanced care planning, and coordination with interdisciplinary teams.<br>This is not a contract role it is a long-term, permanent opportunity.<br><b>Schedule </b><br>• Monday Friday<br>• 8:00am 4:30pm (flexibility based on patient scheduling)<br>• No call<br>• No weekends<br><b>Patient Model </b><br>• In-home visits (must be comfortable traveling locally)<br>• Travel radius up to 90 minutes within market<br>• Most patients clustered within core territory<br>• Average 3 4 patient visits per day<br>• Visit duration may extend up to 90 minutes<br><b>Clinical Focus </b><br>• End-stage renal disease (ESRD)<br>• Diabetes<br>• COPD<br>• CHF<br>• Advanced chronic illness<br><b>Responsibilities </b><br>• Conduct in-home palliative assessments and evaluations<br>• Develop and manage individualized care plans<br>• Manage pain and complex symptom control<br>• Guide patients and families through goals-of-care discussions<br>• Document advanced directives<br>• Collaborate with interdisciplinary team (RN, Social Worker, Chaplain)<br>• Coordinate with primary and specialty providers<br><b>Minimum Requirements </b><br>• Active New Jersey NP license<br>• Active, unrestricted DEA<br>• Board Certified NP (AANP or ANCC)<br>• Minimum 2 years recent palliative care experience (required)<br>• Ability to practice autonomously<br>• Comfortable with home-based care model<br>Preferred:<br>• AGACNP, AGNP, or FNP<br>• Experience working with nephrology/ESRD patients<br><b>Compensation & Benefits </b><br>Competitive base salary (aligned with market benchmarks).<br>Compensation may include performance incentives based on clinical outcomes.<br>Comprehensive benefits package includes:<br>• Medical, dental, vision<br>• 401(k) with employer contribution<br>• Paid vacation and holidays<br>• Paid malpractice coverage<br>• CME allowance<br>• Company-provided equipment (laptop)<br><b>Why Join </b><br>• Mission-driven palliative model<br>• Lower productivity expectations than traditional PCP roles<br>• Relationship-focused patient care<br>• Interdisciplinary support structure<br>• Growing, expanding market<br><b>Application Instructions </b><br>Please submit your CV via DocCafe or directly for consideration.
