<b>It's more than a career, it's a calling. </b><br>MO-SSM Health Saint Louis University Hospital 1201 Grand<br><b>Worker Type: </b><br>PRN<br><b>Job Summary: </b> <br>Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns.<br><b>Job Responsibilities and Requirements: </b> <br>PRIMARY RESPONSIBILITIES<ul><li>Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable. Documents medication related plans and interventions in electronic medical record. </li><li>Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners. Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration. </li><li>Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients. This includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system. </li><li>Precept pharmacy students and residents, as applicable. </li><li>Works in a constant state of alertness and safe manner. </li><li>Performs other duties as assigned. </li> </ul>EDUCATION<ul><li>Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy </li> </ul>EXPERIENCE<ul><li>No experience required </li> </ul> PHYSICAL REQUIREMENTS<ul><li>Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. </li><li>Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements. </li><li>Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. </li><li>Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc. </li><li>Frequent keyboard use/data entry. </li><li>Occasional bending, stooping, kneeling, squatting, twisting and gripping. </li><li>Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. </li><li>Rare climbing. </li> </ul> REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS <br>Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment.
