Primary Care Opportunity in Tucson, Arizona $325Kannual income + $100K Sign on Bonus Role: <ul> <li>Compensation: Base $270,000 (negotiable) + up to 20% of base salary in bonus </li><li>$100,000 sign on bonus </li><li>Primary Care Outpatient </li><li>Value Based Care Model </li><li>Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.) </li><li>Average patients per day: 10-12 </li><li>Panel size: Max of 650 </li><li>Shifts: Monday-Friday 8a-5p </li><li>Call Schedule: Based on number of providers in the market (if 32 physicians in market call 1:32wks) </li><li>Physicians have their own care team with MA/scheduler/ referral specialist and full support from leadership </li> </ul> Benefits: <ul> <li>Health, Vision, Dental (including family options) & paid parental leave program </li><li>Pre-tax or post-tax Roth contributions available.
