This is not an all inclusive list of our service area.*</span></div><div> </div><div><b>Northern: </b>Northbrook, Arlington Heights, Buffalo Grove, Bloomingdale, Schaumburg, Des Plaines</div><div><b>North West: </b>Wheaton, Lombard, West Chicago, Warrenville, Aurora, Batavia</div><div><b>North East: </b>Glenview, Evanston, Norridge, Oak Park, Bellwood, Cicero</div><div><b>Southern: </b>Southern Suburbs, East Chicago, Chicago (South), The Loop, Cicero</div><div><br /><b>Baylor Home Health Physical Therapist Overview:</b></div><div> </div><div><div>Baylor Program opportunities is for those able to dedicate <b>EVERY OTHER</b> Saturday and Sunday </div><div>Main focus will be Starts of Care (SOCs)</div><div>Bonus pay of $50 per completed SOC visit </div><div>Mileage is paid door-to-door</div></div><ul><li><b>Competitive Compensation: </b>Earn between $75 and $150 per visit (+ mileage)</li><li>1:1 therapist-to-patient ratios where you focus on delivering exceptional, personalized care</li><li>Flexible scheduling, variety of assignments, and work-life balance</li><li>Access to online training, CEU opportunities, and tuition discounts through Rasmussen University</li></ul><div><b>Your Role and Responsibilities as a Home Health Physical Therapist:</b></div><ul><li>Provide physical therapy to patients who have lost mobility, strength or functionality in facilities such as nursing homes, rehab centers, assisted living facilities, clinics and hospitals</li><li>Work with a team of physicians, nurses, CNAs, OTs and SLPs to provide excellent patient care</li><li>Assist physician in evaluation of patient function and performance, as well as equipment needs</li><li>Conduct assessments, document progress and determine if therapy services should be modified</li><li>Assist patient with exercises and hands-on therapy to manage pain and increase mobility</li><li>Educate patients and family on established goals, exercises and self-care to ensure progress</li></ul><div><b>Qualifications for an Interim HealthCare® Physical Therapist:</b></div><ul><li>Graduate of an accredited Physical Therapist Program with an active PT license in IL</li><li>Minimum 1 years of physical therapy experience, ideally in home healthcare</li><li>Current CPR and BLS certifications </li><li>Knowledge of state and federal home health regulations</li><li>Strong clinical judgment, excellent communication skills, and a compassionate approach</li></ul><div><br /><b>Why Join Interim?</b><br />As the nation's first home care company, founded in 1966, Interim HealthCare® is seeking dedicated Home Health Physical Therapists (PTs) who are passionate about providing high-quality, personalized care.
