That is why Liberty Hospice provides our hospice patients with state-of-the-art care and pain management services, delivered by our specially trained staff with emphasis on strength, dignity and compassion. <br /> <br /> We are currently seeking an experienced:<br /> <br /> <strong>HOSPICE LICENSED PRACTICAL NURSE (LPN)</strong></p> <p><strong>Part Time</strong><br /> <br /> Job Description:</p> <p><br /> · Provides direct patient care, to include using a variety of skills and necessary coordination of services.<br /> · Completes appropriate documentation as required by policy and procedure and regulation.<br /> · Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.<br /> · Coordinates Hospice services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.<br /> · Accepts and transcribes physician’s orders as directed.<br /> · Forwards all clinical, progress, and summary notes to the Hospice Registered Nurse for coordination of care.<br /> · Attends and participates in IDT meetings.<br /> <br /> Job Requirements:</p> <p><br /> · Successful completion of nursing education from an approved school of nursing. <br /> · Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.<br /> · Ability to independently organize work and provide effective health care services to hospice patients. <br /> · Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.<br /> · Effective communication skills (oral and written). <br /> · CPR certification.<br /> <br /> Visit www.libertyhomecare.com for more information.<br /> Background checks/drug-free workplace.<br /> EOE.</p> </div> </div><br /><br /><br /><img src="https://www.click2apply.net/v/6a4506f86bf8b905ba203850" /><br /><br /><p>PIb60f9371333c-36205-40662212</p>
