The practice is seeking a board-certified Nurse Practitioner with a background in Primary Care/ Adult Medicine or Geriatrics to join their team.<br/><br/>The organization operates under a value-based care model emphasizing preventive medicine, chronic disease management, care coordination, and quality performance metrics.
