Wound Care Nurse Practitioner (NP)<br><br>Heritage Wound Management Specialists (HWMS)<br><br>Location/s: Raleigh-Durham Greater Triangle Area or Eastern North Carolina (Wilson, Greenville, Goldsboro, Kinston) Area<br><br>Job Type: Full-Time | Mobile | Home-Based<br><br>Pay: $110,000.00 - $140,000.00 per year<br><br>Job description / Position Overview<br><br>We are seeking an APP who is looking for something different than the traditional hospital or corporate healthcare environment. This role offers a high degree of clinical autonomy, flexibility, and independence. As a Wound Care provider with HWMS, you will manage your own patient panel, structure your daily schedule, and provide care in a way that prioritizes outcomes. This is an ideal role for a provider who is confident in their clinical decision-making, values patient relationships, and prefers a more flexible and self-directed work environment.<br><br>What Makes This Role Different<br><br>Unlike large health systems or corporate medical groups, HWMS offers:<br><br>* Autonomy in Clinical Practice - You will evaluate, diagnose, and treat patients independently, with support from experienced wound care leadership when needed.<br>* Flexible Scheduling - Your day is not dictated by rigid clinic templates. You will organize your schedule based on patient needs, geography, and clinical priorities.<br>* Lower Administrative Burden - We minimize unnecessary bureaucracy so you can focus on patient care rather than navigating layers of approval.<br>* Patient-Centered Care Model - You will have the time and flexibility to truly manage complex wounds and build meaningful relationships with patients.<br>* Mobile Care Delivery - Care is provided in the home and facility setting, eliminating many of the barriers patients face when accessing traditional wound clinics.<br><br>Care Coordination<br><br>* Collaborate with home health agencies, skilled nursing facilities, and referring providers<br>* Communicate treatment plans and updates to the broader care team<br>* Support transitions of care following hospital or SNF discharge<br>* Educate patients and caregivers on wound management and prevention<br><br>Documentation & Compliance<br><br>* Maintain accurate, timely, and compliant documentation<br>* Ensure documentation meets Medicare and payer requirements<br>* Follow HWMS clinical protocols and workflows<br>* Participate in periodic chart reviews and quality initiatives<br><br>Qualifications Required<br><br>* Active, unrestricted North Carolina license<br>* Board Certification (FNP, AGNP, PA-C, or similar)<br>* DEA license (or ability to obtain)<br>* Valid driver's license and reliable transportation<br><br>Preferred<br><br>* Wound care experience or certification (CWOCN, CWON, WCC, or similar)<br>* Experience in home health, SNF, or post-acute care settings<br>* Familiarity with Medicare documentation and billing practices<br><br>Compensation & Benefits<br><br>* $110,000 - $140,000, based on experience<br>* Bonus potential tied to performance and growth<br>* Mileage reimbursement<br>* Malpractice coverage<br>* Continuing education support<br>* Flexible scheduling<br><br>Benefits:<br><br>* 401(k)<br>* 401(k) matching<br>* Dental insurance<br>* Flexible schedule<br>* Flexible spending account<br>* Health insurance<br>* Health savings account<br>* Paid time off<br>* Retirement plan<br>* Tuition reimbursement<br>* Vision insurance<br><br>Work Location: Remote<br><br>*hec*<br><br>#ZREM
