The home health nurse provides pertinent patient education, treatment, and emotional support to help these individuals recover from injuries and illnesses and/or manage chronic disease processes. The home health nurse demonstrates excellent assessment skills and is technically proficient. Requires ability to anticipate and respond to changes in clinical status of patients.</p><p></p><p>Position requires excellent rapport, understanding, and communication with the patients and their family members. A home health nurse is responsible for communicating with doctors and other healthcare professionals in regards to their home health patients, in a consistent and professional manner. Is personally responsible for obtaining and maintaining a current LPN license within the hiring state in which nursing duties are performed and must meet all State Board of Nursing requirements.
