Be the difference</li><li>Implements and manages a specific and comprehensive treatment plan.</li><li>Works under the supervision of a registered physical therapist in delivering patient treatment.</li><li>Demonstrates advanced skills in administering physical therapy procedures.</li><li>Assesses patient response to treatment, modifying when appropriate with input from the supervising therapist.</li><li>Demonstrates awareness and use of proper safety measures regarding patient, others in the treatment area, and himself</li><li>Interacts regularly with others involved in the patient's care to incorporate an interdisciplinary treatment approach for quality, coordinated care. </li><li>Complies with departmental policies for documentation</li><li>Documents all aspects of assessment, planning, treatment, and discharge in the medical record (and in the working PT chart) in a timely, legible manner.</li><li>Long-term and short-term goals are measurable and achievable within a specified amount of time.</li><li>Daily treatment records are noted in the working PT chart on a daily basis and are clear and professional in their terminology.</li><li>Obtains necessary evaluations and re-evaluations in a timely manner. </li><li>Participates in patient discharge preparations.</li><li>Provides Case Managers with appropriate DME referrals in an adequate amount of time in which the Case Manager is able to secure the equipment for the patient prior to D/C from BEH.</li><li>Provides information regarding follow-up services which may include a referral to other community resources. </li><li>Participates in patient and family education process.</li><li>Provides education and training to patients, families, and caregivers regarding diagnoses, progress, long-term functional outcomes.</li><li>Documents patient/family education appropriately</li><li>Supports and provides educational material to patients/families for support groups as well as community resources. </li><li>Strives to meet department productivity standards.</li><li>Assists with coverage on other units when caseload permits and when multiple caseloads may be modified easily to permit one person to assist with coverage in another area.</li><li>Complies with established standards of efficient patient scheduling to maintain appropriate levels of productivity when covering the inpatient unit. </li><li>Attends staff meetings, team meetings, and mandatory in-services as scheduled.</li><li>Completes mandatory hospital training annually and in a timely manner.</li><li>Completes TB test procedures by the end of the employee's birth month.</li><li>Completes assignments in a timely and thorough manner.</li><li>Assists with care and maintenance of departmental equipment and supplies.</li><li>May be requested to help with routine clean up duties at day's end as needed.</li><li>Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
