margin-right: 0in; margin-bottom: 0in; font-size: 12pt; font-family: Arial, sans-serif;"><span style="font-size: 10.0pt;">Reports all interventions made to the clinical manager and documents findings electronically.</span></li><li style="margin-top: 0in; margin-right: 0in; margin-bottom: 0in; font-size: 12pt; font-family: Arial, sans-serif;"><span style="font-size: 10.0pt;">Collaborates with the primary care physician and clinical team to perform a full patient assessment by gathering the patient’s medical history and medication information.</span></li><li style="margin-top: 0in; margin-right: 0in; margin-bottom: 0in; font-size: 12pt; font-family: Arial, sans-serif;"><span style="font-size: 10.0pt;">Supervises the initial home patient assessment to ensure the HHA is following VNS guidelines and regulations.</span></li><li style="margin-top: 0in; margin-right: 0in; margin-bottom: 0in; font-size: 12pt; font-family: Arial, sans-serif;"><span style="font-size: 10.0pt;">Identifies continued training needs of the Home Health Aide and documents the information electronically.</span></li><li style="margin-top: 0in; margin-right: 0in; margin-bottom: 0in; font-size: 12pt; font-family: Arial, sans-serif;"><span style="font-size: 10.0pt;">Delivers the start of care packet and provides an overall review with the patient/caregiver on initial visit.</span></li><li style="margin-top: 0in; margin-right: 0in; margin-bottom: 0in; font-size: 12pt; font-family: Arial, sans-serif;"><span style="font-size: 10.0pt;">Educates and reviews the paraprofessional plan of care with the Home Health Aide (HHA).</span></li><li style="margin-top: 0in; margin-right: 0in; margin-bottom: 0in; font-size: 12pt; font-family: Arial, sans-serif;"><span style="font-size: 10.0pt;">Participates in quality assurance and education programs as requested by management.</span></li><li style="margin-top: 0in;
