<strong>Overview </strong><br><p> VNS Health RN Field Supervisors are responsible for conducting initial and ongoing in-home supervisory visits to patients, ensuring high-quality care delivery and compliance with clinical standards. This role involves patient assessment, care coordination, caregiver training, and effective communication with the interdisciplinary care team </p><br><p><strong> What We Provide </strong> </p><ul><li> Referral bonus opportunities </li><li> Generous paid time off (PTO), starting at 30 days of paid time off and 9 company holidays </li><li> Health insurance plan for you and your loved ones, Medical, Dental, Vision, Life Disability </li><li> Employer-matched retirement saving funds </li><li> Personal and financial wellness programs </li><li> Pre-tax flexible spending accounts (FSAs) for healthcare and dependent care </li><li> Generous tuition reimbursement for qualifying degrees </li><li> Opportunities for professional growth and career advancement </li><li> Internal mobility, generous tuition reimbursement, CEU credits, and advancement opportunities </li> </ul> <p><strong> What You Will Do </strong> </p><ul><li> Performs initial and ongoing supervisory field visits to patients' homes to provide assessment, training support, in-home coordination and/or crisis management. </li><li> Conveys any significant changes in the patient's condition, emergency intervention, or care plan changes. </li><li> Reports all interventions made to the clinical manager and documents findings electronically. </li><li> Collaborates with the primary care physician and clinical team to perform a full patient assessment by gathering the patient's medical history and medication information. </li><li> Supervises the initial home patient assessment to ensure the HHA is following VNS guidelines and regulations. </li><li> Identifies continued training needs of the Home Health Aide and documents the information electronically. </li><li> Delivers the start of care packet and provides an overall review with the patient/caregiver on initial visit. </li><li> Educates and reviews the paraprofessional plan of care with the Home Health Aide (HHA). </li><li> Participates in quality assurance and education programs as requested by management. </li><li> Participates in special projects and performs other duties as assigned. </li> </ul> <br><strong>Qualifications </strong><br><p><strong> Licenses and Certifications: </strong> </p><ul><li> License and current registration to practice as a Registered Professional Nurse in the State of New York required </li><li> CPR/BLS Certification required </li> </ul> <p><strong> Education: </strong> </p><ul><li> Associate's degree in nursing from an approved program required </li><li> Bachelor's degree in nursing preferred </li> </ul> <p><strong> Work Experience: </strong> </p><ul><li> Minimum of two years' clinical home care experience as Registered Nurse required </li><li> Experience in a licensed home care service agency or CHHA settings preferred </li> </ul> <br><strong>Pay Range </strong><br>USD $77,200.00 - USD $96,500.00 /Yr. <br><strong>About Us </strong><br>VNS Health has been committed to meeting the needs of New Yorkers for over 130 years.
