Job description
Help patients regain independence and return to the daily activities that bring meaning to their lives. As a Home Health Occupational Therapist, you'll work one-on-one with patients in their homes, using creativity, clinical expertise, and problem-solving to help them rebuild the skills needed for everyday living. This role is ideal for a licensed Occupational Therapist who thrives in a flexible, patient-centered environment and enjoys tailoring care to each individual's unique goals and home setting. Working closely with patients, families, and an interdisciplinary care team, you'll provide education, hands-on training, and encouragement as patients progress toward greater confidence and independence in their daily lives.
Office Location: 1919 E. 52nd Street, Indianapolis, IN 46205 Main Service Area Options:
This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes
Monday to Friday, 8 a.m. Weekend On-Call Rotation
Graduate of an accredited occupational therapy program (ACOTE accredited)
Current state license as an Occupational Therapist
Minimum 1 year of experience as a licensed Occupational Therapist
Willingness to travel daily for home visits within service area
Home health experience
Experience with adaptive equipment and home modifications
Knowledge of Medicare home health documentation
Strong clinical assessment and treatment planning abilities
Knowledge of medical terminology and home health regulations
Excellent problem-solving and creative thinking skills
Ability to work independently with minimal supervision
