We re seeking <strong>Nurse Practitioners or Physician Assistants (Advanced Practice Provider) </strong> who thrive in high-touch, patient-centered care and enjoys making a lasting impact on the lives of elderly patients. </p><p><strong>About the Role </strong> </p><p>As an APP on our team, you ll serve as the clinical lead for your patients delivering in-home primary, urgent, and palliative care. You ll work closely with physicians, social workers, paramedics, and care coordinators to ensure each patient receives thoughtful, personalized care. This is a full-time role with a schedule of Monday through Friday, <strong>no weekend work </strong> required. </p><p><strong>Key Responsibilities </strong> </p><ul><li><p>Deliver <strong>primary and palliative care </strong> to elderly patients in their homes (3 6 patients per day). </p> </li><li><p>Perform physical assessments, develop care plans, and manage chronic and acute conditions. </p> </li><li><p>Provide <strong>education and counseling </strong> to patients and families. </p> </li><li><p>Manage medications, labs, imaging orders, and referrals as needed. </p> </li><li><p>Participate in <strong>interdisciplinary team meetings </strong> and care plan reviews. </p> </li><li><p>Maintain meticulous documentation to meet care quality and compliance standards. </p> </li> </ul><p><strong>Experience </strong> </p><ul><li><p>5+ years of progressive NP or APP clinical experience </p> </li><li><p>Strong background in <strong>geriatrics, internal medicine, palliative care, or end-of-life care </strong> </p> </li><li><p>Home-based care experience preferred but not required </p> </li> </ul>
