WCS Healthcare is actively hiring experienced Registered Nurses (RNs) with UAS Assessment experience for multiple openings throughout Manhattan.

This is an excellent opportunity for nurses who enjoy working independently, building relationships with patients in the community, and maintaining a healthy work-life balance through a flexible hybrid schedule.

Whether you're based in Midtown, Lower Manhattan, or Upper Manhattan, you'll be assigned a territory that helps minimize travel and allows you to spend more time focusing on patient care.

What You'll Do

As a UAS Assessment Nurse, you'll help ensure patients receive the appropriate home care services and support needed to safely remain in their homes.

Responsibilities:

• Conduct UAS-NY assessments in patients' homes throughout your assigned territory
• Evaluate physical, cognitive, functional, and psychosocial needs
• Complete accurate and timely assessment documentation
• Determine eligibility for home care and long-term care services
• Collaborate with physicians, care managers, and interdisciplinary healthcare teams
• Educate patients and families on available programs and resources
• Maintain compliance with agency policies and New York State regulations

Qualifications

Required:

Active New York State RN License

Current UAS-NY Certification

Previous UAS Assessment experience

Strong clinical assessment and documentation skills

Preferred:

• Home Care experience
• MLTC experience
• Case Management experience
• Community Health experience

Schedule

• Full-Time
• Monday-Friday
• 9:00 AM-5:00 PM
• 3 days conducting assessments in the field
• 2 days working remotely from home
