Home-Based Palliative Care NP — Autonomous, Patient-Centered

Empatia, part of Gentiva, seeks a compassionate Palliative Care Nurse Practitioner to deliver advanced home-based palliative care for high-quality life. You will work autonomously with the Palliative Medical Director, guiding patients and families through treatment decisions.

You will assess complex needs, develop individualized plans, and ensure compliant documentation, while participating in evening/weekend rotations and collaborating with interdisciplinary teams.
