In-Home Advanced Practice Clinician (CAW) for Senior Care

Alignment Health is seeking an Advanced Practice Clinician for the Care Anywhere program to deliver in-home medical and social support, reducing hospitalizations and disease progression. The role supports seniors, including geriatric patients, within a fast-growing home-care model.

Requirements include a Master’s degree in an NP or PA program, active NP or PA licensure (RN/Furnishing number, board certification), and strong communication and organizational skills.
