Remote Cardiac NP/PA – In-Home Heart Care Leader

Monogram Health seeks a Cardiac Advanced Practice Provider to deliver in-home, evidence-based cardiac care for patients with heart failure. You will assess patients, order tests, prescribe medications, document findings, and collaborate with cardiologists and field-based staff.
