Remote NP/PA for Medicare Wellness & ACP (Flexible Hours)

PharmD Live is seeking a dedicated Nurse Practitioner or Physician Assistant for a fully remote role serving Medicare patients. You will conduct virtual Annual Wellness Visits and Advanced Care Planning sessions with flexible scheduling.

The position requires Hawaii licensure (NP or PA-C) and at least 3 years of clinical experience in AWVs/ACP, with strong Medicare knowledge and excellent telehealth communication. Remote work supports balanced, patient-centered care.
