Remote Nurse Practitioner — Chronic Care & Heart Failure

Cadence is hiring a Remote Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey.

You will lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends.
