Remote SNP Care Management Lead

Alignment Health is seeking a seasoned Manager, Case Management – SNP to steer a remote care management team serving Medicare Advantage SNP members. You will ensure CMS Model of Care compliance while driving quality, efficiency, and member-centered outcomes across a high-performing team.

The role requires 5+ years in clinical case management, SNP experience, and leadership capability. A California RN license is required, with opportunities to expand licensure to other markets.
