A healthcare organization in New York City is currently seeking an experienced, licensed Registered Nurse (RN) to join their team as a Care Manager.

• Patient Population: Adults & Geriatrics

Responsibilities:

The Care Manager (RN) will:

• Provide case management to members, including medical, social, psychological, and physical.
• Develop, implement, and monitor the care plan to help meet the member's needs.
• Perform other duties, as needed.

Qualifications:

• 2+ years of Case Management and/or Home Care experience
• Graduate of an accredited School of Nursing
• NYS Registered Nurse (RN) license

Desired Skills:

• Bachelor's Degree in Nursing
• Bilingual (English and Cantonese, Mandarin, or Spanish)
