Senior Remote Integrated Clinic Pharmacist

Conviva Senior Primary Care is seeking a Clinical Pharmacist for a remote role within the Humana Primary Care Organization. You will support the full physician panel, collaborate with care teams, and participate in high‑risk rounds to optimize pharmacotherapy.

The ideal candidate has a Pharm.D. or equivalent, 5+ years of practice, and licensure in Florida with willingness to obtain additional state licenses. Excellent communication and EMR documentation are required.
