Travel Nurse Practitioner — In-Home Health Evaluations

Signify Health, part of CVS Health, seeks a Full Time Clinician (Nurse Practitioner) to provide in-home health evaluations across a defined coverage area. You’ll travel up to 100% of the time, perform 45-minute visits including medication and medical history review, and work with a national network of clinicians.

Responsibilities include conducting diagnostic/preventive services as indicated, using an iPad workflow, and coordinating with health plans to connect members with the right care.
