Job description
- Start: ASAP
- Role: Care Manager / Acute Care Manager
- Primary function: Care coordination for acute-care patients, including coordinating services, discharge planning, collaborating with physicians/nurses/social workers, and helping ensure patients receive appropriate care across the continuum.
- Work schedule: 5-day workweek. The department operates 7 days a week, so your days off will vary and may not always be consecutive.
- Patient ratio: Approximately 1 Care Manager : 25 patients
- Floating: Must be willing to float to the Laguna facility.
- Dress code: Turquoise scrubs or business casual
- RTO: Maximum 7 days of requested time off.
- Minimum 2 years of acute medical Care Manager experience
- Must have experience specifically performing care coordination.
- Travel experience is mandatory.
- California RN/state license
A pending CA license may be considered during submission, but the license must be active before the assignment begins. - Acute-care experience
Experience in an acute hospital environment is required.

Hourly Pay Rate
W2 Pay Rate RT (Regular Time) $25.00
W2 Pay Rate OT (Over Time) $37.50
Weekly Pay Package (Taxable + Nontaxable)
> W2 Wage (RT+OT based on expected hrs) $1,000.00
> Nontaxable Weekly PerDiem $1,700.00
Total Weekly Gross Pay Package $2,700.00
