Requirements

Responsibilities

Evaluate and treat patients in a skilled nursing facility setting
Develop and implement individualized physical therapy treatment plans
Document patient progress and response to treatment accurately and in a timely manner
Collaborate with interdisciplinary healthcare team to deliver comprehensive care
Educate patients and caregivers on exercises, safety, and home programs
Maintain compliance with facility standards and state/federal regulations

Requirements

Active PT license in NJ required to start the assignment
Doctorate of Physical Therapy (DPT) from an accredited program
BLS/CPR certification
Prior SNF experience preferred
