Key Responsibilities - Develop and implement individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and restoring muscle function according to each patient's unique capabilities. - Oversee and collaborate with Physical Therapy Assistants to ensure the execution of the prescribed plan of care, while providing guidance and support. - Conduct thorough evaluations of patients' functional levels using diagnostic assessments, ensuring strategic adjustments to treatment plans as needed. - Assess the home environments of clients when applicable, making recommendations to promote independence and enhance recovery. - Provide in-home physical therapy services, delivering therapy as prescribed by physicians, while also engaging in manual therapy techniques and functional exercises. - Monitor and document changes in patient conditions and responses to therapy, making appropriate recommendations to revise treatment plans when necessary. - Participate actively in interdisciplinary team meetings, sharing insights and collaborating on holistic care approaches to treatment. - Instruct clients and their families on the effective use and care of physical therapy devices, promoting education around the entire physical therapy process. - Prepare and maintain accurate clinical records, including initial evaluations, progress notes, and discharge summaries to ensure continuity of care. - Stay accessible to Physical Therapy Assistants and provide support during treatment sessions as required.
